Our Rochdale Connecting YouNew teams are in the heart of the community to help you live a healthier and better life. Teams of community connectors are already providing support at public locations across Heywood, Middleton and Rochdale as well as hosting drop-in sessions at four specific ‘hubs’.

The teams can help you with things like employment and education advice, housing, money management, accessing services such as health screening and immunisation, or if you wish to lead a healthier lifestyle by stopping smoking, losing weight or exercising more.

The community connectors can provide information, advice and signposting and if required a referral to an appropriate team or organisation to support you to achieve this.

You can drop in and see them at these locations and times;

  • Rochdale Infirmary on Mondays 1pm – 5pm and Thursdays 8.30am – 12.30pm
  • Phoenix Centre, Heywood on Mondays 8.30am – 12.30pm and Thursdays 1pm – 5pm
  • Middleton Health Centre on Tuesdays 8.30am – 12.30pm and Fridays 1pm – 5pm
  • Littleborough Health Centre on Wednesdays 1pm – 5pm and Fridays 8.30am – 12.30pm

Please note the community connectors service will close at 5pm on Monday 24 December 2018 and re-open at 8.30am on Wednesday 2 January 2019.
